## Abstract

Tool and method for creating a perforation or cavity in a bone or bone structure in contact with a membrane, the tool including a drill having a free end and a channel adapted for supplying a liquid under pressure to the free end, the tool further including a source of the liquid under pressure such that the liquid flows partially into and through the bone or bone structure toward the membrane while drilling the bone or bone structure with the drill, which allows to perform a progressive detachment of the membrane when the drill is close to the membrane while still being inside the bone or bone structure, and before completely piercing the bone or bone structure.

## Claims

### Claim 1

Tool for creating a perforation or cavity in a bone or bone structure in contact with a membrane, said tool comprising a drill having a free end and a channel adapted for supplying a liquid under pressure to said free end, said tool further comprising a source of said liquid under pressure to enable said liquid to flow partially into and through the bone or bone structure toward the membrane while drilling the bone or bone structure with the drill, and to cause a progressive detachment of the membrane when the drill is close to the membrane while still being inside the bone or bone structure, and before completely piercing the bone or bone structure.

### Claim 2 (depends on 1)

Tool according to claim 1, wherein said membrane is a sinus membrane.

### Claim 3 (depends on 1)

Tool according to claim 1, further comprising a flow adapter for diverting a portion of said liquid after entering said channel of said drill, such that a second flow of said liquid is supplied to said free end of said drill that is smaller than or equal to a first flow of said liquid entering said channel.

### Claim 4 (depends on 3)

Tool according to claim 3, wherein said membrane is a sinus membrane.

### Claim 5 (depends on 1)

Tool according to claim 1, further comprising an insert adapted to be inserted into said channel of said drill and for conveying said liquid from said source of said liquid to said channel, wherein an outer diameter of said insert is smaller than an inner diameter of said channel, thus creating an open space between said insert and said channel to accommodate backflow of a portion of said liquid supplied to said channel.

### Claim 6 (depends on 1)

Tool according to claim 1, wherein said drill has an outside surface including a drainage gutter for liquid evacuation.

### Claim 7 (depends on 1)

Tool according to claim 1, wherein said drill comprises a transverse opening connecting said channel to an outside surface of said drill, to enable liquid evacuation.

### Claim 8 (depends on 1)

Tool according to claim 1, wherein said drill comprises an anti-blocking gutter at its free end that prevents said free end from being blocked when drilling said bone or bone structure.

### Claim 9 (depends on 1)

Tool according to claim 1, wherein said free end of said drill comprises a plurality of outlet openings for said liquid.

### Claim 10 (depends on 1)

Tool according to claim 1, wherein said free end of said drill comprises a cutting element.

### Claim 11 (depends on 1)

Tool according to claim 1, wherein said drill comprises a stop.

### Claim 12 (depends on 1)

Tool according to claim 1, wherein said drill has an outer surface extending parallel to a symmetry axis of said drill, and wherein said outer surface is smooth.

### Claim 13

Method for creating a perforation or cavity in a bone or bone structure in contact with a membrane, the method comprising:
drilling said bone or bone structure by a drill; and
providing a liquid under pressure at a free end and through a channel of said drill such that said liquid flows partially into and through the bone or bone structure toward the membrane while drilling the bone or bone structure with the drill, and causing a progressive detachment of the membrane when the drill is close to the membrane while still being inside the bone or bone structure, and before completely piercing the bone or bone structure.

### Claim 14 (depends on 13)

Method according to claim 13, wherein said membrane is a sinus membrane.

### Claim 15 (depends on 13)

Method according to claim 13, further comprising:
providing a first flow of said liquid at an entrance of said channel;
diverting a portion of said first flow, thus creating a second flow of said liquid provided to said free end of said drill, wherein said second flow is smaller than or equal to said first flow.

### Claim 16 (depends on 13)

Method according to claim 13, further comprising:
providing said liquid to said channel of said drill through an insert inserted into said channel;
flowing back, through an open space between said insert and said channel, a portion of said liquid provided to said channel.

### Claim 17 (depends on 13)

Method according to claim 13, further comprising:
drilling said bone or bone structure by said drill until a stop of said drill contacts said bone or bone structure, thus drilling up to a first drilling depth; and
subsequently further drilling said bone or bone structure by a second drill until a second stop of said second drill contacts said bone or bone structure, thus drilling up to a second drilling depth deeper than said first drilling depth.

### Claim 18 (depends on 17)

Method according to claim 17, further comprising:
providing a first flow of said liquid at an entrance of said channel of said drill;
diverting a portion of said first flow, thus creating a second flow of said liquid provided to said free end of said drill, wherein said second flow is smaller than or equal to said first flow;
providing a third flow of said liquid at an entrance of a channel of said second drill during drilling;
diverting a second portion of said third flow, thus creating a fourth flow of said liquid to a free end of said second drill, wherein said fourth flow is smaller than said third flow and smaller than said second flow.

### Claim 19 (depends on 17)

Kit of drills for creating a perforation or cavity in a bone or bone structure according to the method of claim 17.

### Claim 20

Kit of drills for creating a perforation or cavity in a bone or bone structure in contact with a membrane, said kit comprising a plurality of drills including a first drill and a second drill, each drill of said plurality of drills having a free end and a channel adapted for supplying a liquid under pressure to said free end, said channel having an inner diameter, each said drill being adapted for receiving an insert for supplying said liquid to said channel, wherein an outer diameter of said insert is smaller than an inner diameter of said channel, thus creating an open space between said insert and said channel for backflow of a portion of said liquid supplied to said channel; wherein said first drill has a first drilling length and said second drill has a second drilling length longer than the first drilling length, and wherein said first drill has a first inner diameter of said channel and said second drill has a second inner diameter of said channel larger than said first diameter.
