## Abstract

The invention provides methods and devices for creating a localized pleurodesis between the thoracic wall and the lung such that a ventilation bypass conduit may be introduced into the lung through the thoracic wall and visceral membrane without causing a pneumothorax. A catheter or implantable device delivers the pleurodesis agent to the pleural cavity.

## Claims

### Claim 1

A method to introduce a conduit through a parietal membrane and a visceral membrane and into a lung of a subject without allowing air to enter an intrapleural space around the lung, the method comprising:
(a) introducing an implantable medical device which comprises a chemical agent into the intrapleural space;
(b) allowing the release of the chemical agent from the implantable medical device into a localized area of the intrapleural space such that the chemical agent creates a localized adhesion between the parietal membrane and visceral membrane within the localized area; and
(c) subsequent to steps (a) and (b) introducing the conduit from a position external to the lung into tissue of the lung through the parietal membrane and the visceral membrane within the localized adhesion.

### Claim 2 (depends on 1)

The method of claim 1, further comprising removing the implantable medical device before step (c).

### Claim 3 (depends on 1)

The method of claim 1, wherein the implantable medical device comprises a radiopaque marker and wherein step (a) comprises:
(a1) introducing the implantable medical device into the intrapleural space;
(a2) scanning the subject to determine information about a location of the radiopaque marker; and
(a3) positioning the implantable medical device using the information about the location of the radiopaque marker.

### Claim 4 (depends on 1)

The method of claim 1, wherein the chemical agent comprises a radiopaque constituent and wherein step (a) comprises:
(a1) introducing the implantable medical device into the intrapleural space;
(a2) scanning the subject to determine information about a location of the radiopaque constituent; and
(a3) positioning the implantable device using the information about the location of the radiopaque marker.

### Claim 5 (depends on 1)

The method of claim 1, wherein the implantable medical device is biodegradable and wherein step (b) comprises allowing the chemical agent to elute from the implantable medical device as the implantable medical device dissolves.

### Claim 6 (depends on 1)

The method of claim 1, wherein the implantable medical device is non-biodegradable and wherein step (b) comprises (b) allowing the release of the chemical agent from the implantable medical device into a localized area of the intrapleural space such that the chemical agent creates a localized adhesion incorporating the implantable medical device between the parietal membrane and visceral membrane within the localized area.

### Claim 7

A method for treating a lung of a patient using a ventilation bypass system comprising a conduit having a distal end for insertion in the lung through a thoracic wall, a parietal membrane and a visceral membrane wherein the method comprises:
(a) inserting an implantable disk including a pleurodesis agent into an intrapleural space;
(b) delivering the pleurodesis agent to a localized area of the intrapleural space using the implantable disk so as to create a localized pleurodesis between the parietal membrane and the visceral membrane;
(c) creating an opening in the thoracic wall, parietal membrane and visceral membrane through the localized pleurodesis; and
(d) inserting the distal end of the conduit into the lung through the opening in the thoracic wall, parietal membrane and visceral membrane through the localized pleurodesis.

### Claim 8 (depends on 7)

The method of claim 7, further comprising removing the implantable disk after delivering the pleurodesis agent.

### Claim 9 (depends on 7)

The method of claim 7, wherein the implantable disk comprises a radiopaque marker and wherein step (a) comprises:
(a1) introducing the implantable disk into the intrapleural space;
(a2) scanning the subject to determine information about the location of the radiopaque marker; and
(a3) positioning the implantable disk using the information about the location of the radiopaque marker.

### Claim 10 (depends on 7)

The method of claim 7, wherein the pleurodesis agent comprises a radiopaque constituent and wherein step (b) comprises:
(b1) scanning the subject to determine information about a location of the radiopaque constituent; and
(b2) using the information about the location of the radiopaque constituent to confirm the location of the pleurodesis agent.

### Claim 11 (depends on 7)

The method of claim 7, wherein the implantable disk is biodegradable and wherein step (b) comprises allowing the chemical agent to elute from the implantable disk as the implantable disk dissolves.

### Claim 12 (depends on 7)

The method of claim 7, wherein the implantable disk is non-biodegradable and wherein step (b) comprises allowing the release of the chemical agent from the implantable disk into a localized area of the intrapleural space such that the chemical agent creates a localized adhesion incorporating the implantable disk between the parietal membrane and visceral membrane within the localized area.

### Claim 13

A method to introduce a conduit through a parietal membrane and a visceral membrane and into a lung of a subject without allowing air to enter an intrapleural space around the lung, the method comprising:
(a) introducing an implantable disk which comprises a chemical agent into the intrapleural space;
(b) allowing the release of the chemical agent from the implantable disk into a localized area of the intrapleural space such that the chemical agent creates a localized adhesion between the parietal membrane and visceral membrane within the localized area; and
(c) subsequent to steps (a) and (b) introducing the conduit from a position external to the lung into tissue of the lung through the parietal membrane and the visceral membrane within the localized adhesion.
