Abstract
Disclosed is an instrument and method for use in a procedure to effect anastomosis of a patient's bladder and urethra following a prostatectomy. The instrument comprises a tube assembly, and an end effector assembly operably supported thereby, where the end effector assembly includes a positioner assembly and an anchor driver assembly in operable mechanical communication with a handle, for use in performing anastomotic procedures.
Claims10
An instrument for use in a procedure to effect anastomosis of a patient's bladder and urethra following a prostatectomy, comprising:
- a tube assembly, comprising a spine member, a first rod, and a second rod, wherein said rods are movable with respect to each other and to said spine member;
- a handle, wherein said handle is coupled with said tube assembly;
- and
- an end effector assembly operably supported by said tube assembly, further comprising:
- a positioner assembly in operable mechanical communication with said first rod so as to be actuable by movement of said first rod with respect to said spine member;
- an anchor driver assembly in operable mechanical communication with said second rod so as to be actuable by movement of said second rod with respect to said spine member;
- and
- at least one anchor releasably held by said anchor driver assembly for insertion from within the bladder lumen through the bladder wall and into the pelvic floor.
A method for effecting the anastomosis of a patient's bladder and pelvic floor following a prostatectomy, comprising the steps of:
- providing an anastomotic instrument comprising:
- a handle, wherein said handle comprises a hand support;
- a tube assembly connected to said handle and comprising a bend angle;
- an end effector assembly connected to said tube assembly, wherein said end effector assembly is in operable mechanical communication with said handle via said tube assembly;
- inserting said end effector assembly in a retrograde direction into the patient's urethra and bladder;
- opening a positioner provided on said end effector assembly within the patient's bladder;
- grasping said handle proximate to said hand support;
- using said handle and said hand support to manipulate said instrument, wherein said tube assembly is urged against the patient's pubic arch such that the pubic arch is used as a fulcrum about which said tube assembly is manipulated, where the movement of said tube assembly about said fulcrum causes said positioner to urge the patient's bladder wall towards the patient's pelvic floor;
- and
- using said handle to further actuate said anastomotic instrument.
