Improved therapeutic uses for N-terminal BPI protein products are described in patients that have BPI-reactive anti-neutrophil cytoplasmic antibodies.

In a method of treating a subject with a bactericidal/permeability-increasing (BPI) protein product, the improvement comprising administering an N-terminal BPI protein product to a subject having non-N-terminal-BPI-reactive antibodies.
