The present invention is directed to compositions comprising heme-containing components and to methods that stimulate erythropoiesis comprising administration of a composition of the invention, such as purified hemoglobin. Surprisingly, heme-containing components such as hemoglobin can induce erythropoiesis in the presence of decreased concentrations of erythropoietin (Epo) and, in fact, functions synergistically with Epo. Further, heme-containing components can be used alone or in conjunction with Epo for the treatment of anemias and other disorders due to decreases in erythropoietin or iron. It has been discovered that, in the presence of Epo, both hemoglobin and cross-linked hemoglobin can compensate for the reduction in erythroid cell growth and differentiation that occurs in the presence of reduced concentrations of Epo. The effect is specific to erythropoiesis, as evidenced by a lack of growth of non-erythroid progenitors such as CFU-GM.
Use of a hemoglobin composition in the preparation of a medicament for administration to a patient, to increase hemoglobin synthesis in the patient, in the presence of an amount of erythropoietin which is itself ineffective in stimulating normal hemoglobin synthesis in the patient.
Use according to any preceding claim wherein the patient is a human.
Use according to any preceding claim wherein the increased synthesis of hemoglobin is both increased adult hemoglobin synthesis and increased fetal hemoglobin synthesis.
Use according to any preceding claim wherein the hemoglobin composition comprises natural hemoglobin. recombinant hemoglobin, cross-linked hemoglobin, oxy hemoglobin, deoxy hemoglobin, met hemoglobin, NO-hemoglobin, CO-hemoglobin or ferric hemoglobin.
Use according to any preceding claim wherein the hemoglobin composition comprises hemoglobin and erythropoietin (Epo), the amount of Epo in said composition being insufficient by itself to cause increase in hemoglobin, in the absence of the hemoglobin of said composition.
Use according to any preceding claim wherein the hemoglobin composition is specific for the stimulation of erythropoiesis in erythroid progenitors.
Use according to any preceding claim wherein the hemoglobin composition does not significantly stimulate the proliferation or differentiation of non-erythroid cells.
Use according to any preceding claim of the composition for preparation of a medicament for administration to a human patient having a peripheral blood Epo content less than 5 mU/ml.
Use according to any preceding claim of said hemoglobin composition for preparation of a medicament for administration to a patient undergoing cell transplantation.
Use according to any preceding claim of said hemoglobin composition for preparation of a medicament for treatment of porphyria in a human patient.
A process of stimulating primitive stem cells that are characterized by the presence of the hematopoietic progenitor marker CD34 and lacking either CD38 or CD33 differentiation antigens, which comprises culturing said stem cells in the presence of an effective amount of added hemoglobin.
A process of increasing human and fetal hemoglobin synthesis in erythroid colonies which comprises culturing erythroid colonies in vitro in the presence of added amounts of hemoglobin.
A composition comprising hemoglobin covalently coupled to erythropoietin.
