Abstract
A method for onsite hemorrhage control in trauma patients using a portable rapid autotransfusion device can involve recovering a first portion of patient blood from an extravascular space into a fluid reservoir of the device. A negative internal pressure can be applied to the blood. The blood can be conditioned, such as by oxygenating and removing carbon dioxide. The conditioned blood can be returned to the patient intravenously at a rate that matches the rate of blood recovery, ensuring that the net volume of returned blood is maintained substantially equal to the net volume of removed blood.
Claims18
A portable rapid autotransfusion system for point-of-injury or similar onsite portable hemorrhage control in a trauma patient, the system comprising:
- a fluid reservoir, at least partially open to air in an ambient environment about the system, the fluid reservoir configured for recovering a first portion of patient blood, from an extravascular space of a patient into which the first portion of the patient blood has hemorrhaged;
- and
- an enclosed flexible bag, fluidly connected to an outlet of the fluid reservoir configured for permitting outgassing of gas from the first portion of patient blood out of the enclosed flexible bag;
- a first conditioning fluid circuit, including or fluidly connected to the enclosed flexible bag and fluidly sealed from the air in the ambient environment about the system, the first conditioning fluid circuit including:
- a first blood conditioner including a first oxygenator, the first blood conditioner configured for conditioning the first portion of patient blood including oxygenating, and removing carbon dioxide (CO2) from the first portion of patient blood;
- and
- a heat exchanger for controlling, concurrently with the conditioning, a temperature of the first portion of patient blood;
- and
- a second conditioning fluid circuit, fluidly connected to an outlet of the first blood conditioner, the second conditioning fluid circuit including a second oxygenator;
- wherein, concurrent with recovering patient blood from the patient, the second conditioning circuit is configured to return conditioned blood back to the patient intravenously via a fluid outlet of the second conditioning fluid circuit.
A method for point-of-injury or similar onsite portable hemorrhage control in a trauma patient, the method comprising:
- recovering a first portion of patient blood, from an extravascular space of a patient into which the first portion of the patient blood has hemorrhaged via a fluid reservoir, wherein the fluid reservoir is at least partially open to air in an ambient environment;
- outgassing air from the first portion of patient blood, within an enclosed flexible bag fluid connected to an outlet of the fluid reservoir, the outgassing including impeding blood from permeating through the enclosed flexible bag;
- conditioning the first portion of patient blood via a first conditioning fluid circuit fluidly connected to the enclosed flexible bag and fluidly sealed from air from an external ambient environment, the conditioning including oxygenating the first portion of patient blood via a first oxygenator and controlling a temperature of the first portion of patient blood;
- and
- conditioning the first portion of patient blood via a second conditioning fluid circuit, following the conditioning via the first conditioning fluid circuit, including oxygenating the first portion of blood via a second oxygenator;
- and
- returning, concurrent with recovering patient blood from the patient, conditioned blood back to the patient intravenously via a fluid outlet of the second conditioning fluid circuit.
