A photopolymer composition for hologram formation, comprising:

• a polymer matrix comprising a siloxane-based polymer and a (meth)acrylate polymer containing one or more reactive functional groups in the side chains;
• a photoreactive monomer;
• and
• a photoinitiator,
• wherein the one or more reactive functional groups comprise a functional group represented by the following Chemical Formula 2:
• wherein, in the Chemical Formula 2,
• L1 is an oxyalkylene group having 1 to 10 carbon atoms, an oxyalkenylene group having 2 to 20 carbon atoms, or an oxyalkynylene group having 2 to 20 carbon atoms.
