Abstract
Systems and methods for cardiac pacing are provided, where a pacing lead is placed at or near the bundle of His. A method for pacing a heart of a patient comprises: introducing a sheath to vasculature of the patient; steering the sheath within a coronary sinus in the heart to lodge a distal end of the sheath to a target location proximal to the bundle of His above a septum separating a left ventricle and a right ventricle of the heart; advancing a pacing lead through a lumen of the sheath to the target location; coupling the pacing lead to cardiac tissue at the target location; removing the sheath; and electrically pacing the bundle of His using the pacing lead.
Claims20
A pacing device for pacing the heart of a patient, comprising:
- (i) an encasement including therein a pulse generator;
- and
- (ii) a pacing electrode protruding from a distal end of the encasement, wherein the pacing electrode is electrically coupled to the pulse generator and comprises:
- (a) a linear segment extending at a distal tip of the pacing electrode, wherein the linear segment is configured to pierce cardiac tissue at a target location to a first depth of the cardiac tissue for testing the placement of the pacing electrode at the target location, and
- (b) a helical segment at a proximal end of the linear needle, wherein the helical segment is configured to screw into the target location, thereby advancing the linear needle to a second depth of the cardiac tissue and anchoring the pacing electrode to the target location.
A method for pacing a heart of a patient, the method comprising:
- (a) introducing a sheath into the right atrium of the heart of the patient;
- (b) advancing a distal end of a pacing electrode through a lumen of the sheath to a target location, the distal end of the pacing electrode comprising:
- (i) a linear segment extending at a distal tip of the pacing electrode, wherein the linear segment is configured to pierce cardiac tissue to a first depth of the cardiac tissue for testing the placement of the pacing electrode at the target location, and
- (ii) a helical segment at a proximal end of the linear needle, wherein the helical segment is configured to screw into the target location, thereby advancing the linear needle to a second depth of the cardiac tissue and anchoring the pacing electrode to the target location;
- (c) (i) coupling the linear segment of the pacing electrode to a cardiac tissue at the target location, (ii) activating the pacing electrode to generate a first diagnostic result, (iii) acquiring the first diagnostic result, and (iv) determining whether the pacing electrode at the target location is efficacious based on the first diagnostic result;
- (d) coupling the helical feature of the pacing electrode to the cardiac tissue at the target location if the pacing electrode at the target location is determined to be efficacious based on the first diagnostic result;
- and
- (e) electrically pacing the target location using the pacing electrode.
