## Abstract

Disclosed are methods of transseptal crossing to permit introduction of large bore catheters into to the left atrium, such as to deploy left atrial appendage closure devices or to repair or replace a mitral valve. The method may include the steps of providing a single pass, large bore transseptal crossing system, having a tubular access cannula and an energy delivery wire movably extending through the tubular access cannula. A first electrode tip on the energy delivery wire is brought into contact with a fossa ovalis and energized. The energy delivery wire and cannula are advanced into the left atrium. A large bore sheath is advanced directly over the access cannula and into the left atrium. The cannula and energy delivery wire are removed, and a large bore index procedure catheter may be advanced through the large bore sheath and into the left atrium.

## Claims

### Claim 1

A method of accessing a left atrium, the method comprising the steps of:
providing a single pass, large bore transseptal crossing system having:
a tubular access cannula at least a portion of the tubular access cannula being electrically conductive;
an energy delivery wire comprising an electrically conductive portion, the energy delivery wire configured to be movable through the tubular access cannula, and be independently energized relative to the tubular access cannula;
a first tubular insulation layer surrounding a sidewall of the tubular access cannula that leaves a distal tip of the tubular cannula exposed, the distal tip comprising at least one distal projection; and
a second insulation layer in between the energy delivery wire and the sidewall;
positioning the at least one distal projection of the tubular access cannula in contact with a fossa ovalis;
energizing the energy delivery wire to deliver energy from the energy delivery wire and to the fossa ovalis and to enable passage into the left atrium;
after energizing the energy delivery wire, advancing the energy delivery wire through the fossa ovalis;
after advancing the energy delivery wire, energizing the at least one distal projection of the tubular access cannula to deliver energy from the tubular access cannula to the fossa ovalis to enlarge a passage into the left atrium created by the energy delivery wire and to enable the tubular access cannula to advance into the left atrium; and
after energizing the tubular access cannula, advancing a large bore sheath directly over the tubular access cannula and into the left atrium.

### Claim 2 (depends on 1)

A method of accessing the left atrium as in claim 1, wherein the electrically conductive portion of the tubular access cannula comprises a first electrode tip, wherein the first electrode tip comprises the at least one distal projection, and wherein energizing the tubular access cannula comprises energizing the first electrode tip prior to passage of the tubular access cannula into the left atrium.

### Claim 3 (depends on 1)

A method of accessing the left atrium as in claim 1, wherein energizing the tubular access cannula comprises energizing the tubular access cannula in a monopolar mode.

### Claim 4 (depends on 1)

A method of accessing the left atrium as in claim 1, further comprising the steps of:
removing the tubular access cannula and energy delivery wire from the large bore sheath, and
advancing an index procedure catheter through the large bore sheath and into the left atrium.

### Claim 5 (depends on 4)

A method of accessing the left atrium as in claim 4, further comprising the step of advancing a left atrial appendage implant deployment catheter through the large bore sheath and into the left atrium.

### Claim 6 (depends on 4)

A method of accessing the left atrium as in claim 4, further comprising the step of advancing a mitral valve repair catheter through the large bore sheath and into the left atrium.

### Claim 7 (depends on 4)

A method of accessing the left atrium as in claim 4, further comprising the step of advancing a mitral valve replacement catheter through the large bore sheath and into the left atrium.

### Claim 8 (depends on 1)

A method of accessing the left atrium as in claim 1, wherein the energy delivery wire has an outside diameter of about 0.035 inches.

### Claim 9 (depends on 1)

A method of accessing the left atrium as in claim 1, wherein the tubular access cannula has an outside diameter of about 0.050 inches.

### Claim 10 (depends on 1)

A method of accessing the left atrium as in claim 1, wherein the electrically conductive portion of the tubular access cannula comprises a first electrode tip, wherein the first electrode tip comprises the at least one distal projection, and wherein the electrically conductive portion of the energy delivery wire comprises a second electrode on a distal tip of the energy delivery wire.
