## Abstract

An ostomy base plate includes an adhesive layer, a top layer, and a sensor assembly. The adhesive layer is adapted to adhere the ostomy base plate to skin of a user. The top layer is on a distal side of the ostomy base plate and has a top layer opening. The sensor assembly is disposed between the adhesive layer and the top layer and includes electrodes with electrical connection parts. The electrical connection parts of the electrodes extend through the top layer opening for electrical connection with a removable data monitor. The sensor assembly is adapted to monitor a state of condition of the adhesive layer.

## Claims

### Claim 1

An ostomy base plate comprising:
an adhesive layer on a proximal surface of the ostomy base plate, where the adhesive layer includes a stoma opening and is adapted to adhere the ostomy base plate to skin of a user;
a top layer on a distal side of the ostomy base plate, the top layer provided with a top layer opening; and
a sensor assembly disposed between the adhesive layer and the top layer, the sensor assembly including a plurality of electrodes providing first electrode parts and second electrode parts, with the second electrode parts distributed around the stoma opening and the first electrode parts extending from the second electrode parts to a terminal end having electrical connection parts;
wherein the first electrode parts of the plurality of electrodes of the sensor assembly extend through the top layer opening in the top layer;
wherein the first electrode parts comprise a flexible circuit with the terminal end located on a distal surface of the top layer and the terminal end is movable to bend away from the distal surface of the top layer to allow a removable data monitor to access and electrically connect to the terminal end;
wherein the sensor assembly is adapted to communicate to the removable data monitor information related to a state of condition of the adhesive layer.

### Claim 2 (depends on 1)

The ostomy base plate of claim 1, wherein the sensor assembly is adapted to communicate to the removable data monitor information related to erosion of the adhesive layer.

### Claim 3 (depends on 1)

The ostomy base plate of claim 1, wherein the sensor assembly is adapted to communicate to the removable data monitor information related to leakage of stomal output relative to the adhesive layer.

### Claim 4 (depends on 1)

The ostomy base plate of claim 1, wherein the sensor assembly is adapted to communicate to the removable data monitor information related to a classification of stomal output leakage conditions relative to the adhesive layer.

### Claim 5 (depends on 1)
