## CPC

- A61B 17/704: {the longitudinal element passing through a ball-joint in the screw head}
- A61B 17/7032: {Screws or hooks with U-shaped head or back through which longitudinal rods pass}
- A61B: DIAGNOSIS; SURGERY; IDENTIFICATION (usefulness limited to only animals A61D)

## Abstract

An implant assembly includes a screw body, including anchor portion and proximal head portion, and a cradle movably mounted in the screw body to allow for controlled angulation between a spinal connection element disposed in the cradle and the screw body. The cradle is pivotable in one or more selected directions about one or more axes relative to the screw body. The cradle may be generally, substantially spherical in shape and allow for unrestricted movement in one or more directions around one or more axis.

## Claims

### Claim 1

An implant assembly comprising:
a proximal head portion;
a substantially-spherical cradle having a central bore configured to receive a spinal fixation element and opposed flat portions on an external portion thereof, the cradle seated within the proximal head portion so that the opposed flat portions bind the cradle against the proximal head portion;
a locking mechanism receivable in the proximal head portion; and
an anchor portion extending distally from the proximal head portion, the anchor portion configured to engage with bone;
wherein actuation of the locking mechanism is effective to fix an orientation of the cradle relative to the proximal head portion.

### Claim 2 (depends on 1)

The implant assembly of claim 1, wherein the locking mechanism is a set screw, and wherein the cradle is configured to allow monoaxial movement of the spinal fixation element relative to the proximal head portion prior to being locked by the set screw.

### Claim 3 (depends on 1)

The implant assembly of claim 1, wherein the cradle includes:
a lower element defining a lower portion of the central bore; and
an upper element defining an upper portion of the central bore.

### Claim 4 (depends on 3)

The implant assembly of claim 3, wherein the opposed flat portions are defined by the lower element and the upper element.

### Claim 5 (depends on 3)

The implant assembly of claim 3, wherein the lower element is configured to mate to the upper element to form the substantially-spherical cradle.

### Claim 6 (depends on 5)

The implant assembly of claim 5, wherein the lower element and the upper element include an interlocking surface configuration to assist in the mating thereof.

### Claim 7 (depends on 1)

The implant assembly of claim 1, wherein the cradle comprises first and second halves that are configured to be mated to each other to define the central bore.

### Claim 8 (depends on 1)

The implant assembly of claim 1, wherein the proximal head portion includes a U-shaped slot for receiving the cradle.

### Claim 9

An implant assembly, comprising:
a proximal head portion defining a cavity;
a substantially-spherical cradle disposed within the cavity, the substantially-spherical cradle having opposed flat portions on an external portion thereof and defining a central bore, the cradle being configured to rotate within the cavity about the opposed flat portions;
a locking mechanism receivable within a proximal end of the proximal head portion; and
an anchor portion extending distally from the proximal head portion, the anchor portion being configured to be driven into bone;
wherein the central bore of the cradle is sized to receive and retain a spinal rod; and
wherein the locking mechanism is configured to fix the cradle at a first orientation relative to the proximal head portion such that a spinal rod received within the central bore is fixed relative to the proximal head portion.

### Claim 10 (depends on 9)

The implant assembly of claim 9, wherein the proximal head portion is U-shaped and configured to receive a spinal rod through the proximal end thereof.

### Claim 11 (depends on 9)

The implant assembly of claim 9, wherein the cradle includes a first portion and a second portion, and wherein the first and second portions are configured to couple together to define the central bore.

### Claim 12 (depends on 11)

The implant assembly of claim 11, wherein the opposed flat portions are defined by the first portion and the second portion.

### Claim 13 (depends on 11)

The implant assembly of claim 11, wherein the lower element is configured to mate to the upper element to form the substantially-spherical cradle.

### Claim 14 (depends on 9)

The implant assembly of claim 9, wherein the central bore is chamfered to facilitate rotation of the spinal rod when the spinal rod is received therein.

### Claim 15 (depends on 9)

The implant assembly of claim 9, wherein the locking mechanism is a set screw, and wherein the cradle is configured to rotate within the central cavity about only a single axis prior to being locked by the set screw.

### Claim 16 (depends on 9)

The implant assembly of claim 9, wherein the lower element and the upper element include an interlocking surface configuration to assist in the mating thereof.
