## Abstract

A polyaxial fastener is secured within an implant with a retaining ring. The ring is assembled onto a circumferential groove formed in the fastener head. The groove and ring are positionable, when the fastener is installed into an implant, at a wide portion of the mating polyaxial aperture in the implant. The ring is compressible into the groove to form a narrowed diameter, which is passable into a narrow diameter of the aperture, near an entrance to the aperture. The ring is released to expand and increase an overall diameter of the head and ring assembly, thereby rending the assembly too wide to pass by the narrow entrance to the aperture, thereby securing the assembly within the implant.

## Claims

### Claim 1

A method of surgery comprising:
abutting a plate system against two or more vertebrae, wherein the plate system comprises:
a plate comprising a first aperture for receiving a first fastener and a second aperture for receiving a second fastener; and
a retaining pin;
inserting a first fastener through the first aperture, wherein the first fastener comprises a head portion and an extension portion, wherein the head portion comprises a groove and a retaining ring received in the groove, wherein the retaining ring helps to prevent back out of the first fastener from the plate, wherein the retaining ring comprises an engagement notch and the first fastener comprises an access port, and wherein the retaining pin is configured to be received in the access port and the engagement notch to retain the ring in a compressed configuration.

### Claim 2 (depends on 1)

The method of surgery of claim 1, wherein the retaining ring comprises a fixed ring end and a movable ring end.

### Claim 3 (depends on 2)

The method of surgery of claim 2, wherein the movable ring end comprises the engagement notch.

### Claim 4 (depends on 3)

The method of surgery of claim 3, wherein the head portion of the first fastener comprises the access port.

### Claim 5 (depends on 4)

The method of surgery of claim 4, wherein in the compressed configuration the engagement notch of the retaining ring and the access portion of the first fastener are aligned.

### Claim 6 (depends on 1)

The method of surgery of claim 1, wherein the retaining pin comprises a cam member.

### Claim 7 (depends on 1)

The method of surgery of claim 1, wherein the retaining ring has a pair of ends such that the retaining ring does not form a complete diameter.

### Claim 8 (depends on 7)

The method of surgery of claim 7, wherein the pair of ends of the retaining ring comprises a fixed ring end and a movable ring end.

### Claim 9

A method of surgery comprising:
abutting a plate system against two or more vertebrae, wherein the plate system comprises:
a plate comprising a first aperture for receiving a first fastener and a second aperture for receiving a second fastener; and
a retaining pin;
inserting a first fastener extendable through the first aperture, wherein the first fastener comprises a head portion with a retaining ring extending around the head portion, wherein the retaining ring helps to prevent back out of the first fastener from the plate, wherein the retaining ring comprises an engagement notch and the first fastener comprises an access port, and wherein the retaining pin is received in the access port and the engagement notch to retain the ring in a compressed configuration; and
inserting a retaining pin through the retaining ring.

### Claim 10 (depends on 9)

The method of surgery of claim 9, wherein the retaining ring comprises a fixed ring end and a movable ring end.

### Claim 11 (depends on 10)

The method of surgery of claim 10, wherein the movable ring end comprises the engagement notch.

### Claim 12 (depends on 11)

The method of surgery of claim 11, wherein the head portion of the first fastener comprises the access port.

### Claim 13 (depends on 12)

The method of surgery of claim 12, wherein in the compressed configuration the engagement notch of the retaining ring and the access portion of the first fastener are aligned.

### Claim 14 (depends on 9)

The method of surgery of claim 9, wherein the plate comprises third and fourth apertures.

### Claim 15 (depends on 9)

The method of surgery of claim 9, wherein the retaining pin comprises a cam member.

### Claim 16 (depends on 9)

The method of surgery of claim 9, wherein the retaining ring has a pair of ends such that the retaining ring does not form a complete diameter.

### Claim 17 (depends on 16)

The method of surgery of claim 16, wherein the pair of ends of the retaining ring comprises a fixed ring end and a movable ring end.
